hereby declare that
1) I have been acquainted with the general terms and conditions and the operational rules for the use of the fitness center Dream Gym s.r.o., Company ID: 05 222 681, address of the establishment Štefánikova 1303, Mladá Boleslav, and I have thoroughly informed the Minor about the rules contained in these documents,
2) I agree that the Minor may use the fitness center Dream Gym s.r.o.,
3) I am aware that a condition for using the fitness center is reaching the age of 18 and I acknowledge the possible health and safety risks for the Minor, for which I bear all possible responsibility,
4) the Minor is capable of controlling their actions and assessing their consequences, and therefore Dream Gym s.r.o. does not need to exercise any supervision over them. In the event that the Minor causes any harm, the Minor will be responsible for it, as will I, as their Legal Representative,
5) I will fulfill all monetary obligations that arise for the Minor from the general terms and conditions and the operational (visiting) rules, and which the Minor does not fulfill properly and on time, particularly I will pay any contractual penalty, compensation for harm, or unjust enrichment on their behalf,
6) I am aware that Dream Gym s.r.o. processes my personal data as the Legal Representative of the Minor, for the purposes of concluding and managing the contract, as well as for fulfilling its obligations, which are further specified in the personal data processing policy,
7) I make this statement also as an authorized representative of the second legal representative of the Minor, on whose behalf I join all the points mentioned above.
